The "LMHPCO Member Directory" ("the Directory") is compiled and published by Louisiana-Mississippi Hospice and Palliative Care Organization ("LMHPCO") as a reference source of demographic and professional information on individual hospices in Louisiana and Mississippi. Use of the Directory is subject to the Terms and Conditions of Use for LMHPCO.org, and also subject to the following:
The Directory may be used by the general public to allow quick access to information about LMHPCO member hospices. However, the Directory is not a referral service. For further information about a LMHPCO member hospice, contact the member hospice itself as well as the applicable state licensing agency and state or national association of hospice agencies.
Additional information is available exclusively to LMHPCO member hospices who have successfully logged in to access LMHPCO.org Members Only content. (See LMHPCO Member Login.)
The use of the Directory by third parties to verify information about, or the credentials of, hospices -- including but not limited to healthcare entities, hospitals, insurance companies, and/or managed care organizations -- is not sufficient to meet the standards of any regulatory agency, licensing agency or credentialing body, such as the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the National Committee for Quality Assurance (NCQA) or others. The Directory does not contain sufficient information with which to verify hospice providers' credentials. This information is provided as a service to our members.
The Directory and/or any of its data, listings or other constituent elements may not be downloaded, republished, resold or duplicated, in whole or in part, for commercial or any other purposes, or for purposes of compiling mailing lists or any other lists of hospices. The use of the Directory to establish independent data files or compendiums of statistical information is prohibited.
Disclaimer of Warranties and Liabilities
Although LMHPCO undertakes reasonable efforts to keep the information contained in the Directory accurate, the information is not guaranteed and no responsibility is assumed for errors or omissions. LMHPCO does not warrant the accuracy, completeness, timeliness or merchantability or fitness for a particular purpose of the information contained in the Directory; nor does LMHPCO in any way endorse the individuals or hospice agencies described in the Directory or the care or services they may provide.
In no event shall LMHPCO be liable to you or anyone else for any decision made or action taken by you in reliance on such information. Any damages for any reason shall be limited to the amount paid to access the Directory, if any.
