1. Provider Member (Voting)
Any hospice licensed by the State of Louisiana or Mississippi and whose interests are consistent with our purposes and mission may apply for membership with LMHPCO. A provider member hospice shall be granted one (1) vote.
Provider-Membership dues are based on the number of all new hospice patients admitted under the same State License in the previous calendar year, regardless of reimbursement. The number of physical office locations is not considered when determining the total number of admissions. Each separately State Licensed Provider must have a separate LMHPCO Provider Membership. Note: Each program's patient total information will remain confidential and will not be disclosed in any form. The first physical location of the Provider will be assessed $800.00. Each additional physical location of the Provider will be assessed $250.00 per location.
Dues are $800 annually, plus $250.00 for each additional physical location, plus $2 per new admission, up to a maximum of 350 new admissions per year. First-time Provider-Member dues are $250.00 for the first year of operation or during the initial year of the formation of the hospice. (Although full payment of annual dues is preferred, quarterly installments are acceptable by checking the appropriate space below. Quarterly payments are due: January 1; April 1; July 1; and October 1 each year.)
2. Associate Member (Non-Voting)
Suppliers, vendors, and companies that support end of life care across Louisiana and/or Mississippi, and are interested in supporting the purpose and goals of LMHPCO. Associate Members are included in the annual Membership Directory, are listed on our website, and receive the monthly LMHPCO Newsletter. Associate Members are also entitled to membership rates when attending all workshops sponsored by LMHPCO throughout the year. Associate Member dues are $300 annually.
3. Organizational Member (Non-Voting)
End-of-Life care organizations, educational institutions, homecare organizations, nursing facilities, home health agencies, and other organizations that provide or support end of life care in Louisiana and/or Mississippi and are interested in supporting the purpose and goals of LMHPCO. Organizational Members are included in the annual Membership Directory, are listed on our website, and receive the monthly LMHPCO newsletter. Associate Members are also entitled to membership rates when attending all workshops sponsored by LMHPCO throughout the year. Organizational Member dues are $200 annually.
4. Professional Member (Non-Voting)
Physicians, Pharmacists, Nurses, Social Workers, CPAs, Attorneys, Consultants, Professors, Educators, and other professionals who are involved in end of life care in Louisiana and/or Mississippi and are interested in supporting the purpose and goals of LMHPCO. Professional Members are included in the annual Membership Directory, are listed on our website, and receive the monthly LMHPCO newsletter. Professional Members are also entitled to membership rates when attending all workshops sponsored by LMHPCO throughout the year. Professional Member dues are $100 annually.
5. Individual Member (Non-Voting)
Any individual, interested in supporting end of life care and the purpose of LMHPCO. Individual Members receive the monthly LMHPCO newsletter and are entitled to membership rates when attending all workshops sponsored by LMHPCO throughout the year. Individual Member dues are $60 annually.
