Leslie Lancon Memorial Educational Hospice Nursing Scholarship
PURPOSE
To provide scholarships to registered nurses, licensed practical nurses, and nursing assistants who meet the criteria to sit for the National Board for Certification of Hospice and Palliative nurses (NBCHPN) Certification Exam or who are enrolled in an accredited institution of higher learning for advanced practice.
AWARD
Scholarships for cost of the NBCHPN Certification Exam or a tuition stipend.
QUALIFICATIONS
A. NBCHPN Certification:
- An employee in good standing with a minimum of two years of Hospice experience.
- Must submit a copy of the testing company certification acknowledgement letter, indicating that the individual passed the exam.
- May be a new or re-newing certificant.
- Must submit a completed 2005 NBCHPN Certification Exam Scholarship Application to the Education Chair of Louisiana-Mississippi Hospice and Palliative Care Organization (LMHPCO) by the stated deadline.
- Must submit one reference letter from a professional colleague.
- Must submit a reference letter from the employer.
- Must submit a bio-sketch, resume or CV.
B. Scholarship for advanced practice:
- An employee in good standing with a minimum of two years of Hospice nursing experience.
- Must submit proof of enrollment with an institution of higher learning (LPN to RN, ADN to BSN, BSN to MSN, etc.)
- Must submit one reference letter from a professional colleague.
- Must submit a reference letter from the employer.
- Must submit a bio-sketch, resume or CV.
REQUIREMENTS
All responses must be typed or printed and confined to the space provided on Part A of the application form. You may wish to duplicate parts of the form to use as work sheets in preparing application.
DEADLINE DATE
The LMHPCO, regardless of postmark or other circumstances, must receive original plus 3 copies of completed application packet by April 1, 2007. No faxed materials will be accepted. If you have any questions, please contact Jamey Boudreaux, Executive Director.
